Product name, intended use and nominal capacity.
Ingredient names, CAS numbers and percentages by weight, including the propellant.
Contact details for the individual or company applying.
Required supplier, manufacturer or importer details and an emergency contact number.

For a raw material without an independent CAS number, provide an explanation and its supplier SDS.
Available supplier SDSs, test results and reference files.
